Frontier Pediatric Care is committed to serving our entire community. We spend at least 5% of our clinical time on charity care for our community—serving the most vulnerable and those in great need of pediatric care.

We dedicate 15% of our care to reduced-cost services for families who would like the Frontier experience but are not in the financial position to afford the regular membership rate. 

Sliding Scale Specifics:

• For families with a household income of less than $40,000 per year, we offer 60% off the regular membership rate with a family maximum membership fee of $120/month.
• For families with a household income between $40,000 and $60,000 per year, we offer 30% off the regular membership rate with a family maximum membership fee of $240/month.
• For families with a household income greater than $60,000 per year, we offer our regular practice rates with a family maximum membership fee of $300/month.

We require proof of income, which is as simple as a photo confirming last year's tax return or a current pay stub. We review applications monthly and open spots as they become available.
